For orders that are faxed or mailed, please fill out and print the on-line order form and make sure that all of the personalized information is complete and accurate. To do this, select your item(s), fill in the personalized information, color selection (if needed) and then click on Add to Shopping Cart. When you have completed your shopping, click on View Shopping Cart, check your order for accuracy and print out the order form.

Check it again, and then fax or mail this order form with a method of payment to the address or fax number listed above.

Remember, personal checks take an extra 7-10 days to process.
